Trafford Council is seeking a Social Care Assessor to join the Early Intervention and Discharge Social Work Team. The role involves working across acute hospital and community settings to support safe, timely discharge and promote independence with person-centred plans.

You will assess needs, participate in Best Interests discussions, and collaborate with families, carers and multidisciplinary teams while maintaining the individual's voice and outcomes at the center of practice.

Formación

• Experience of working with adults who require health and social care support.
• Understanding of Care Act 2014 and its practical application.
• Ability to carry out strengths-based, outcome-focused conversations.

Responsabilidades

• Undertake Care Act assessments and Best Interests discussions within timescales.
• Work across health and social care to develop proportionate, person-centred care plans.
• Participate in the team’s duty rota and collaborate with colleagues to support safe discharge.
