Bedfordshire Hospitals NHS Foundation Trust is seeking a Trusted Assessor to join hospital discharge teams across Bedfordshire. The role focuses on assessing care needs and arranging appropriate support to ensure safe, timely discharges.

The successful candidate will work with Social Workers, ward staff and care providers, contributing to better outcomes for vulnerable adults in a collaborative, hands-on environment.

Formación

• Experience in adult social care environments such as hospitals, care homes, or domiciliary services.
• Strong written communication and report production skills.
• Ability to work independently and manage competing priorities.
• Comfort communicating with patients, social workers, and care providers.

Responsabilidades

• Undertake person-centred assessments of individuals' care and support needs within hospital settings.
• Produce accurate assessment reports using hospital systems to support care providers.
• Work closely with hospital discharge teams, ward staff, Social Workers and care providers.
• Liaise with care home and domiciliary care managers to understand services available.
• Identify changes in care needs and communicate them to relevant professionals.
• Support timely and safe hospital discharges while keeping individuals’ needs central.
• Maintain accurate records and manage workload across multiple hospital environments.
