Calderdale Metropolitan Borough Council is seeking a Social Care Service Co-ordinator to work with adults with complex needs within the LinC into Calderdale Team in the Adult Services and Wellbeing Directorate. We are looking for a creative and resourceful Social Care Service Co-ordinator to work with us in the LinC into Calderdale Team which is part of the Adult Services and Wellbeing Directorate at Calderdale Council. The team provides guidance and assistance to people who may not meet the Care Act eligibility but who, nevertheless, requires some support and to prevent, reduce or delay the need for stator services.
We work in partnership with a variety of community-based organisations, Health and Public Health partners, to identify and direct people to appropriate place-based service to enable people to live a fulfilled life.
We use strengths-based practice focussing on positive outcomes and being independent and strive to put the person at the heart of assessments.
