Job summary

Cambridgeshire County Council is recruiting three full-time Adult Support Co-Ordinators as part of a 12-month Home Care Framework programme across Cambridgeshire. Based in Huntingdon, you will work with adults to deliver person-centred support, undertake strengths-based reviews, and collaborate with families, carers and providers to achieve positive outcomes.

You will contribute to transforming home care services, ensure accurate records, and support independence and wellbeing.

About the role

We have an exciting opportunity for three full-time experienced and motivated Adult Support Co-Ordinators to join us as part of a dedicated 12‑month programme supporting the implementation of a new Home Care Framework across Cambridgeshire.

This high‑profile programme will play a key role in shaping the future of home care services for adults across the county, ensuring people receive high-quality, person-centred support that promotes independence, wellbeing, and positive outcomes.

As part of this programme, you will undertake reviews with adults currently receiving home care services across the following service areas:

Learning Disabilities

This role provides a unique opportunity to contribute to a significant transformation programme, working directly with individuals, families, carers, providers and partner agencies to ensure support arrangements continue to meet people's needs and aspirations.

You will be joining a dedicated team focused on delivering one of the most important programmes currently underway across adults.

What will you be doing?

You will complete strengths-based reviews and work collaboratively with individuals, families, carers, providers and partner organisations to achieve positive, person-centred outcomes. You will refer individuals to advocacy services where appropriate and ensure care and support plans accurately reflect their current needs, outcomes and goals.

You will support adults to maximise their independence, wellbeing and connections within their communities, identifying opportunities to improve outcomes through strengths-based approaches and personalised support. You will also maintain accurate and timely records in line with statutory and organisational requirements.

As part of the programme team, you will contribute to the successful implementation of the new Home Care Framework across Cambridgeshire, working towards key milestones while maintaining a strong focus on quality and person-centred practice. You will provide support and guidance to individuals as they navigate changes associated with the new framework.
