Milton Keynes University Hospital NHS Foundation Trust is seeking a Virtual Ward Senior Practitioner to lead clinical assessments and care planning for patients supported at home within the virtual ward. You will work across a range of MDTs and health systems to create clinical pathways and ensure proactive management.

The role involves developing pathways with MKUH and the new SDEC centre, coordinating meetings, and monitoring patients remotely (Doccla).

Qualifications

• Advanced clinical assessment skills essential.
• Experience working with multidisciplinary teams and clinicians.
• Excellent communication and empathy with patients and families.

Responsabilités

• Lead and collaborate with MDTs to develop and evaluate clinical management plans.
• Develop clinical pathways with MKUH and the SDEC centre for virtual ward care.
• Coordinate patient pathways and ensure accurate documentation of decisions.
• Remote monitoring of patients across respiratory and frailty pathways using Doccla.
• Ensure effective referral and triage processes for the Virtual Ward.
• Oversee caseload management and ensure high-quality service delivery.
• Provide physical health and biopsychosocial assessments; conduct frailty assessments.
• Liaise with Frailty Team and other agencies to support timely discharge and continuity of care.
• Act as link between the team and external agencies; supervise staff if required.
