Job summary

Procare Health Ltd is seeking a Nights Senior Community Nurse (Band 6) to deliver high-quality nursing care to patients at home during night hours across Guildford and Waverley. The role requires autonomous clinical assessment, triage, and coordination with GPs and hospices to prevent avoidable hospital admissions.

You will work 22 hours per week (2 night shifts), be registered with NMC, and hold a nurse diploma or degree with community nursing experience.

Qualifications

• Registered Level 1 Nurse with current NMC registration.
• Educated to diploma or level 6 / degree in nursing or relevant area.
• Community nursing skills incl. compression bandaging, Dopplers, catheters, syringe drivers and palliative care symptom management.

Responsibilities

• Assess, plan, deliver and evaluate nursing care at patients’ homes during night shifts.
• Triage and prioritise referrals with autonomous clinical decision making.
• Coordinate care with GPs, hospices and multidisciplinary teams; maintain electronic records and contribute to audits.
• Mentor colleagues and students and support staff development.
