Your Impactful Role:

As a member of our team, you’ll play a pivotal role in supporting individuals in transition within our Move on Housing portfolio. You’ll provide crucial medium to low support, helping clients move from high support temporary services to independent living.

Key Responsibilities:
  • Support Work: Embrace a psychologically informed approach, conducting assessments, and empowering clients with practical life skills training.
  • Collaboration: Collaborate with High Support services and external partners to ensure move on opportunities are available
  • Housing Management: Welcome new residents, maintain meticulous records, and ensure smooth void management processes.
  • Monitoring: Keep a pulse on client data, crafting insightful reports on service outcomes, and ensuring financial compliance. Lead on Health & Safety monitoring to ensure compliance
  • Compliance: Champion organisational policies and regulatory requirements, prioritising health and safety, safeguarding, and data management.
  • Equality and Diversity: Foster a culture of inclusivity and equity within our organisation, leading by example and driving positive change.
  • Experience: Previous experience in delivering support to individuals with complex and enduring needs, and the use of risk assessments/support planning tools
  • Knowledge: Good knowledge of mental health wellbeing, addiction, and relapse management are valued
  • Communication: Clear and compassionate communication is key to success in this role.
