Qualifikationen

• Registered practitioner with degree/diploma plus post-registration training or equivalent.
• Membership of the relevant Professional Body.
• Up-to-date portfolio showing ongoing professional development.
• Experience at Practitioner Band 5 level.
• Experience of specialist working.
• Experience using electronic patient/ service user record systems.
• Ability to use own initiative while upholding Trust values.
• Analytical ability to assess situations and determine appropriate action.

Aufgaben

• Provide a high quality, person-centred approach to care delivery.
• Manage a caseload of patients with referrals from GP, 999, hospital wards, NHS 111 and other teams.
• Offer specialist advice on management and care of patients/service users.
• Devise specialist programmes of care for other professionals to deliver.
• Plan, implement and review health improvement programmes.
• Provide supervision, mentorship, training and clinical support to students and practitioners.
• DBS check is required for this post.
