HCRG Care Group is seeking a Band 7 Speech and Language Therapist to join our Early Supported Discharge Stroke Team in Gravesend. You will manage a specialist caseload, delivering assessments, diagnoses and rehabilitation for adults post-stroke within a multidisciplinary team.

You will contribute to service development, support colleagues, and drive quality improvement while providing leadership and training to junior staff in a community setting.

Qualifications

• Degree in Speech and Language Therapy and HCPC registration.
• Membership of the Royal College of Speech and Language Therapists (RCSLT).
• Dysphagia training with ability to independently manage swallowing disorders.
• Significant post‑registration experience with adults following stroke.

Responsabilités

• Lead SLT provision within the Early Supported Discharge pathway for stroke patients.
• Provide specialist assessment, diagnosis and rehabilitation in community settings.
• Develop personalised care plans and support discharge planning.
• Deliver training and supervise junior staff and rehabilitation assistants.
