Job summary

Bridgend County Borough Council is seeking an experienced Social Worker to join the Commissioning for Complex Needs Team. You will support adults with complex needs to build on strengths, work with families, carers and providers to promote progression and independence, and ensure the right accommodation, care and funding are in place.

The role emphasises strengths-based, outcome-focused practice, with hybrid working and regular supervision. Welsh language greeting is a requirement for this post.

Qualifications

• Qualified Social Worker registered with Social Care Wales.
• Experience with adults with complex needs.
• Commitment to strengths-based, person-centred and outcome-focused practice.
• Ability to build positive relationships with individuals, families, providers and partners.
• Knowledge of NHS Continuing Healthcare (CHC) and funding processes.
• Welsh language ability is a requirement for this post.

Responsibilities

• Conduct assessments and reviews to support progression plans.
• Develop progression plans to promote independence and wellbeing.
• Collaborate with individuals, families, carers, providers and partner agencies.
• Support access to NHS Continuing Healthcare funding.
• Contribute to developing solutions for complex needs and future options.
• Maintain strengths-based and outcome-focused practice in all work.
