Job summary

Bridgend County Borough Council is seeking an experienced Social Worker to join the Commissioning for Complex Needs Team. You will support adults with complex needs to build on strengths, work with families, carers and providers to promote progression and independence, and ensure the right accommodation, care and funding are in place.

The role emphasises strengths-based, outcome-focused practice, with hybrid working and regular supervision. Welsh language greeting is a requirement for this post.

Qualifications

  • Qualified Social Worker registered with Social Care Wales.
  • Experience with adults with complex needs.
  • Commitment to strengths-based, person-centred and outcome-focused practice.
  • Ability to build positive relationships with individuals, families, providers and partners.
  • Knowledge of NHS Continuing Healthcare (CHC) and funding processes.
  • Welsh language ability is a requirement for this post.

Responsibilities

About the Role

At the heart of this role is progression.

We believe support should create opportunities for people to progress, build on their strengths and achieve the outcomes that matter most to them.

Using strengths-based and outcome-focused social work practice, you will work alongside individuals, families, carers, providers and partner agencies to develop progression plans that promote independence, wellbeing, choice and control. Bringing professional curiosity, creativity and critical thinking to complex situations, you will help identify opportunities, overcome barriers and support people to achieve their goals and aspirations. Through assessments, reviews and collaborative working, you will help ensure people have the right accommodation, care and support to achieve the outcomes that matter most. You will work closely with providers and partners to ensure support remains focused on progression, enabling people to develop skills, build confidence and move closer to living the life they want to live.

You will also play an important role in supporting access to NHS Continuing Healthcare (CHC) and other funding arrangements, helping to ensure people receive the right support at the right time.

Alongside your direct work with individuals, you will contribute to developing solutions for people with complex needs and help inform future accommodation, care and support options, ensuring services continue to evolve to meet changing needs.

  • Is confident working with complexity, uncertainty and positive risk-taking.
  • Believes in people's strengths and their ability to achieve meaningful change.
