Job summary

A leading company in social care is seeking a qualified Social Worker to coordinate care and conduct assessments for vulnerable adults. The role involves working with complex cases in line with the Care Act and related legislation, ensuring comprehensive support for individuals. Qualifications

  • Qualified social worker with a degree in social work.
  • Experience in care coordination and statutory assessments.
  • Knowledge of relevant legal frameworks (e.g., Care Act, Mental Health Act).

Job description

The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.

Social Work Specific:

  • Arranging the provision or purchase of services to meet the assessed needs of individuals and their carers in more complex cases, whether within Gloucestershire or at a distance and to reassess in line with Care Act requirements.
  • Undertake the statutory strengths based assessments to facilitate personalised provision of care and support.
  • Working in accordance with statutory and legal requirements and offering specialist knowledge and consultation to the team on the Care Act, Mental Health Act, Human Rights Act, Housing Act and a range of social care policy issues including appropriate adult and property protection.
  • Social Work provision within GHC Trust is currently undergoing service development to strengthen the Social Care management and service provision whilst maintaining integration alongside the development of a new Social Care Hub.
