Job summary

Barnet Council is seeking a Lead Practitioner Transitions Team to promote learning and professional development across Communities, Adults and Health and to provide supervision and high professional standards in social care.

You will support people to maintain health, wellbeing and independence by providing information, signposting to local services, and delivering high-quality social work assessments for adults and carers under the Council’s duties. Hybrid working is available.

Qualifications

• Professional qualifications in Social Work and registered with the relevant professional body.
• Experience and training in social work management and leadership.
• Extensive knowledge of social care services and outcomes frameworks.

Responsibilities

• Promote continuous learning and professional development across teams.
• Provide supervision and professional leadership to ensure high standards.
• Assess adults and carers with a focus on social care outcomes under the Care Act 2014.
