Job summary

Bristol City Council’s Young Adults Transitions Service is recruiting an experienced Social Worker / Senior Practitioner to support young people aged 14–25 with additional needs as they move into adulthood.

Based in Bristol City Hall, this 5-month contract offers a mix of in-office and home working, with supervision of a multidisciplinary team and leadership duties within the service. Registration with Social Work England is essential.

Qualifications

• Current registration with Social Work England.
• Experience working with adults or young people with additional needs and supporting transitions into adulthood.
• Experience providing supervision, guidance or professional support to colleagues.

Responsibilities

• Complete Care Act assessments for young people with additional needs transitioning into adulthood.
• Support young people to maximise their independence through assessments, care planning and occupational therapy support.
• Develop support plans and contribute to commissioning of services to meet assessed needs.
• Identify alternative sources of support including charitable organisations and technology to promote independent living.
• Provide day-to-day consultation and supervision to staff across the multidisciplinary team.
• Support the development and improvement of the transitions service alongside Team Managers.
