Job summary

Calderdale and Huddersfield NHS Foundation Trust is recruiting a Senior Rehabilitation Assistant to deliver short, time-limited treatments to adults in their own homes. The role involves protocol-based care, progress tracking for registered staff, and supporting complex cases across Calderdale on a seven-day rota.

Recent rehabilitation or reablement experience is required, with a focus on maintaining independence and preventing hospital admissions.

Qualifications

• GCSE English and Maths (9-2) or equivalent or evidences high literacy and numeracy.
• Competence-based Level 3 qualification in care or Care Certificate.
• Willingness to undertake relevant training or development.

Responsibilities

• Carry out treatment to protocols based on competencies.
• Monitor progress and feed back to the practitioner.
• Make minor adjustments to treatment programmes within set limits.
• Assist registered practitioners with complex treatment.
• Assess for, order and issue basic equipment under protocols.
• Deliver planned, time-limited active therapy and social care support.
• Carry out rapid assessment and crisis intervention for adults 18+ in community settings.
• Support hospital discharges and prevent unnecessary admissions.
• Work in patients' own homes across Calderdale on a seven-day rota.
• Promote independence, dignity and choice.
• Take part in initiatives including Urgent Community Response and Virtual Ward.
