Responsibilities

• Act as the first point of contact for new referrals, ensuring information is recorded accurately and progressed promptly
• Coordinate assessments, calendars, documentation and communication with assessors and relevant stakeholders
• Prepare and update support plans, risk assessments, nomination forms and move-in information
• Maintain complete, confidential and audit-ready service user records
• Produce regular vacancy, capacity, hours and portal updates to agreed schedules
