Job summary

Comfort Care Services in Slough HQ is seeking a Referral & Assessment Coordinator to join the Assessments Team. You will own the administration from initial enquiry through assessment, funding and move-in, working independently with initiative.

The ideal candidate has senior admin experience in health or social care, excellent IT skills (Excel, Word, PowerPoint, Outlook, Teams) and strong communication. A-level English and Maths or related qualification are desirable.

Qualifications

  • Senior administration experience in a demanding but friendly office.
  • Independent worker able to prioritise with minimal supervision.
  • Self-motivation: proactive, organised and proactive following up actions.
  • Excellent IT and systems experience: confident databases, spreadsheets and Microsoft Office.
  • Excellent communication with internal and external stakeholders.
  • Accuracy and judgement handling sensitive information.
  • Sector experience in health care, social care, mental health or supported living desirable.
  • A-level English and Maths or equivalent; admin/health and social care qualification desirable.

Responsibilities

  • Act as the first point of contact for new referrals, ensuring information is recorded accurately and progressed promptly.
  • Coordinate assessments, calendars, documentation and communication with assessors and relevant stakeholders.
  • Prepare and update support plans, risk assessments, compatibility checks, nomination forms and move-in information.
  • Maintain accurate vacancy, capacity, service user, funding and hours information across databases, spreadsheets and internal systems.
  • Follow up referrals, costings and funding decisions, keeping clear records and providing timely updates.
  • Liaise professionally with assessment, operations, care, finance, housing providers and external partners.
  • Maintain complete, confidential and audit-ready service user records.
  • Produce regular vacancy, capacity, hours and portal updates to agreed schedules.

About the opportunity

We at Comfort Care Services are looking for an experienced Senior Administrator to join our Assessments Team as a Referral & Assessment Coordinator. You will take ownership of the administration supporting each referral from initial enquiry through assessment, funding and move-in.

This role will suit someone who works confidently and independently, uses initiative and can keep complex information, deadlines and stakeholders moving without close supervision. Experience within health care, social care, mental health or supported living is highly desirable.

What we are looking for

  • Sector experience: health care, social care, mental health or supported living experience is highly desirable.
  • Education: A-level English and Maths, or equivalent, or above. A relevant administration, business support or health and social care qualification is desirable.
