Job summary

• Support the manager as required, across the range of duties within the post.
• In conjunction with Team Managers, develop efficient pathways of care.
• Work with practice teams to ensure an integrated approach to care.
• Deliver clinical and managerial supervision to junior staff.
• Liaise with other professionals to ensure patients receive appropriate interventions.
• Promote and maintain a multi-disciplinary/multi-agency approach to care.
• Participate in service and multi-agency development.
