Job summary
Cross Gates Primary Care Ltd in East Leeds is seeking an experienced Advanced Clinical Practitioner to lead the Proactive Care Team and shape neighbourhood health as a senior Band 8a role. The post combines expert clinical practice with clinical leadership, operational management, service development and system partnership working.
The postholder will work with the PCN Clinical Director, GP Lead and PCN Manager to develop a coordinated, population-based model of care, delivering proactive care
Qualifications
- Expert knowledge of advanced clinical practice and managing complex conditions.
- Experience leading clinical teams and service development.
- Strong skills in population health management and MDT collaboration.
Responsibilities
- Provide clinical leadership and operational management of the Proactive Care Team.
- Lead delivery of proactive care programmes and frailty pathways.
- Coordinate care home and community care through MDTs and partners.
- Develop neighbourhood health model with cross‑sector collaboration.
- Use data and governance to monitor outcomes and drive improvements.
- Support prevention, early intervention and escalation planning.
