Essex Partnership University NHS Foundation Trust is seeking a Band 7 Primary Care Mental Health Practitioner to join the Primary Care Mental Health Service covering the Braintree and Maldon locality in Essex.

The role is full-time (37.5 hours per week) on a 12-month fixed-term basis. You will triage, assess and provide brief interventions from GP practices, working within multidisciplinary teams to support patients and streamline referrals.

Qualifications

• Triage and assess patients in a community mental health setting.
• Experience in community mental health services preferred.
• Strong communication and risk management skills.

Responsabilités

• Triage and assess patients to direct them to the most appropriate support.
• Provide and undertake physical health investigations such as ECGs and interpret results.
• Act as gatekeeper for referrals into secondary mental health services and advise on care pathways.
• Deliver autonomous clinical assessments and brief interventions within GP practices.
• Work with multidisciplinary teams to improve access to care and outcomes.
