Job summary

Essex Partnership University NHS Foundation Trust is seeking a Senior Community Mental Health Practitioner to join our established Multidisciplinary team. The role focuses on recovery-oriented, personalised care plans for individuals with a range of mental health disorders, from diagnosis to recovery.

You will work within the MDT, act as a Care Co-ordinator, deliver initial assessments, and support timely discharges from hospital.

Qualifications

  • Registered Mental Health Nurse with community mental health experience.
  • Ability to formulate personalised care plans reflecting service user needs and networks.
  • Experience working within a Multidisciplinary Team (MDT) and CPA approach.

Responsibilities

  • Provide and oversee care of service users.
  • Produce accurate documentation and maintain records per policy.
  • Undertake comprehensive assessments and formulate personalised care plans.
  • Conduct risk assessments of individuals and environments.
  • Attend the weekly MDT meeting and liaise with partners.
  • Administer medications and monitor effects in line with policy.
  • Support timely discharges from hospital where appropriate.

Senior Community Mental Health Practitioner | Essex Partnership University NHS Foundation Trust

As part of our Community Transformation programme, we have an opportunity for a Registered Mental Health Nurse to join our established Multidisciplinary team. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders: supporting them from diagnosis to recovery, to enable them to lead the lives they wish to live. You will work with a Multidisciplinary Team: a spectrum of clinical professionals, which includes Nurses, Psychologists, Support Time Recovery workers, etc: as well as a range of collaborative system partners.

We are looking for a positive team player who has a special interest in working compassionately with individuals and their wider support networks.

To work within the MDT and alongside system partners to deliver high quality care and treatment to service users under the care of the specialist community mental health service.

To act as a Care Co-ordinator; formulating care plans, completing/updating risk assessments and collaboratively review care needs via the CPA approach.

To work alongside inpatient services; supporting timely discharges from hospital in keeping with policy and processes.

To fulfil the role of the Duty Worker as part of a service rota.

To undertake Initial Assessments; delivering comprehensive feedback to the MDT and service users/referrers.

  • Responsible for administration of medications, assessment of side effects, review concordance and clinical effectiveness of medication regime in line with Trust policy
  • Undertake Mental Health Nursing Needs Assessments – contributing to Self-Directed Support assessments/packages
