Job summary

Hackney Council's Preparing for Adulthood Service (PfA) invites a qualified social worker to join as Senior Practitioner, supporting practitioners and deputising for the Team Manager within a multidisciplinary team. You will contribute to Care Act assessments, safeguarding, and complex care planning for adults with learning disabilities and autism.

You must be registered with Social Work England, hold post-qualification adult services experience, and provide supervisory support.

Qualifications

• Must be registered Social Worker with Social Work England
• Post-qualification experience in statutory Adult Services
• Experience providing supervisory/management support to social workers
• Experience with learning disabilities and autism preferred
• Commitment to academy approach and integrated health and social care work

Responsibilities

• Provide supervisory support to practitioners and deputise for Team Manager
• Deliver high-quality social work to adults with learning disabilities and complex needs
• Work within a multidisciplinary team including health and social care colleagues
• Lead Care Act assessments, safeguarding and complex court of protection activities where required
