This permanent, full-time role offers a salary ranging from £35,392 to £42,618.
Job summary
NHS Sussex ICB ambition is to create an inclusive working environment which reflects our commitment to celebrate and promote diversity for all groups. We are particularly under-represented in staff from Black, Asian, and Minority Ethnic (BAME) and disabled communities and actively encourage and welcome applications from these communities. Furthermore, we welcome applications from reservists and ex-armed forces as we recognise the benefits of the values, skills, training and experience that they bring to their work with us.
We encourage all applicants to share their equality information with us confidentially during the application process so that we can statistically monitor how fair and equitable our recruitment process is and how well we are reaching applicants from diverse communities and identify where we need to improve. This information is removed before shortlisting.
PLEASE NOTE: Integrated Care Boards (ICBs) are undergoing significant changes. Recently, they have been asked to cut their operating costs by 50%, which includes reducing their workforce. We do not know at this point whether All-Age Continuing Care will be included in these changes and what this may look like.
Main duties of the jobThe post holder will play a key role across the CHC service, working within the remit of the National Framework for Continuing Healthcare, to deliver an effective, efficient and equitable service to the people and populations of Sussex.
Prior experience of CHC would be beneficial, but is not essential for this role, as you will work within an experienced and supportive team who will support you to develop the relevant knowledge required. However, the post holder should have experience of working within health or care, and will need excellent verbal and written communication skills, as you will be required to work closely with patients, families, carers and other external partners.
As a Senior Practitioner, your role will include delivery of a variety of functions across the CHC service, including: Assessment, Reviews, Care Planning and Case Management.
You will also have the opportunity to work across other areas of CHC, including audit, appeals, retrospective assessments, complaints, assessment verification, and many more. You will be supported to develop the necessary skills and experience to work across the whole pathway, safely, confidently and effectively. NHS Sussex is strongly committed to your development, and through a structured 1:1 and appraisal system, will work with you to recognise and create a plan to meet your development needs.
About usThe NHS Sussex Integrated Care Board is a newly created organisation that will oversee the local NHS, support the different NHS partners within it and make decisions on how money is best spent for our populations. NHS Sussex has taken on the commissioning functions previously carried out by Clinical Commissioning Groups (CCGs).
We are proud of our organisational values. Our values underpin how we work together. In recruitment, we seek to attract, select, and induct staff whose personal values are aligned with ours.
NHS Sussex ICB recognises the positive value of diversity, promotes equality and challenges discrimination and we welcome and encourage applications from people of diverse backgrounds and identities. We particularly welcome applications from disabled and Black, Asian, and Minority Ethnic (BAME) candidates as these staff groups are currently under-represented throughout our organisation currently. We have made a positive commitment to employing disabled people and guarantee to interview all disabled candidates who meet the minimum essential criteria for the role as set out in role profiles.
NHS Sussex has committed to achieving Net Zero Carbon emissions by 2045.
The AACC embraces an agile, flexible way of working, recognising that this will lead to a more effective and productive workforce, so full and part time applications are welcomed.
