The relocation package is for anyone joining the Trust in a Clinical role who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

Are you a Registered Learning Disability Nurse looking to progress and develop your career?

The East and South Cheshire Community Learning Disability Team is an established, supportive and progressive team. They are looking to recruit a highly motivated professional to work as a Senior Practitioner (Community Nursing) within the East & South Community Learning Disability Team (CLDT) which covers both the Crewe and Macclesfield areas. The CLDT is an established team comprising of nurses, speech and language therapists, occupational therapists, physiotherapists, psychologists and psychiatry.

Main duties of the job

The Senior Practitioner will work as part of the multi-disciplinary Community Support Team, and will be expected to work autonomously, managing a defined caseload of adult service users with learning disabilities, complex health needs, neurodevelopmental difficulties and /or challenging behaviour.

An excellent understanding of the issues adults with learning disabilities may experience is essential for this post, as is a good understanding of Positive Behaviour Support, Mental Health, Positive Risk Assessment, Care Programme Approach and Case Management.

In addition to direct clinical skill, the successful applicants will be able to demonstrate skills in sharing knowledge through training and consultation and be able to modify and evaluate practice in line with clinical and service needs.

The post holder will be an excellent communicator, have good time management skills and demonstrate a willingness to be an effective and robust team player. They will also need to demonstrate an ability to work across a range of health and social care settings with a clear emphasis on the development and training of others. They would be expected to lead on Quality Improvements initiatives and projects that will have a significant impact on the health and wellbeing of people with a learning disability across East & South Cheshire.

Person specification
Qualifications
  • First level nurse. RNLD/RNMH/ RMN (degree level)
  • Evidence of post registration module training in specialist area to diploma/degree level e.g. physical health in learning disabilities module; epilepsy module; dementia module.
  • Evidence of post registration training in e.g. - autism - epilepsy
  • Demonstrate commitment to ongoing learning & development.
Knowledge
  • Ability to assess/Plan/Evaluate care to clients and carers.
  • Ability to participate in and develop nursing practice.
  • Support clients in crisis.
  • Ability to chair meetings and lead a team e.g. CPA.
  • Deliver training to other professionals and services.
  • Special interests related to current service developments (e.g. epilepsy; autism etc).
Experience
  • A reasonable amount of post qualifying experience of working with adults with a Learning Disability in a community setting
  • Direct experience of working with clients with complex needs i.e. challenging behaviour, epilepsy, mental health problems, continuing health care.
  • Clinical supervision of staff
  • Delivery of training to other staff, service users and carers
  • Positive attitude to people with LD.
  • Awareness of current issues
  • Awareness of relevant legislation and good practice relating to learning disabilities (Valuing People; Mental Health Act; Mansell 3 etc)
  • Experience in delivery of quality initiatives
