The Integrated Urgent Care Team is a joint health and social care service which is leading on the implementation of 'Home First' which is Tameside and Glossop Admission Avoidance and Discharge to Assess Model.
The first part of our Urgent Care service transformation will be based around The Integrated Urgent Crisis Team (IUCT). The service strives to provide high quality community healthcare services across Tameside & Glossop.
