Job summary

Kirkwood Hospice is seeking an experienced Physiotherapist to join our Integrated Community Services team in West Yorkshire. You will deliver specialist physiotherapy across inpatient and community settings, enabling personalised care for patients with life-limiting illnesses.

Qualifications

• Experience in palliative and end-of-life care
• Experience with SystmOne or other electronic records
• Full UK driving license and regular access to a vehicle

Responsibilities

• Autonomously manage an allocated caseload with holistic assessment and timely intervention.
• Provide specialist physiotherapy to people with progressive life-limiting illness across inpatient and community settings.
• Collaborate with the MDT to plan discharge and optimise quality of life for patients and carers.
• Deliver education and training to patients, carers and inter-disciplinary team, supporting service development and quality improvement.
