Colchester Hospital University NHS Foundation Trust is seeking a Band 6 Physiotherapist to join the Reactive Emergency Assessment Community Team (REACT) in Ipswich. The REACT team delivers a multi-disciplinary, integrated community service including Urgent Community Response, OPAT, OOHs and the Frailty Virtual Ward; successful candidates will work across pathways in collaboration with ESNEFT.

Responsabilités

To be an active member of a multi-disciplinary, integrated community team.
To manage a defined caseload of complex individuals, using evidence based/person centred principles.
To assess, plan, implement and evaluate therapy interventions.
To provide leadership within the therapy team
To provide clinical supervision to band 5 physiotherapists and supervise band 3 and 4 support staff.
To effectively triage and schedule all therapy referrals to the service in accordance with Trust's policies and procedures and advise junior colleagues on more complex issues.
To manage a caseload of patients with a range of multiple and complex health needs.
To utilise a high level of clinical knowledge and experience to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.
