Job summary

East Suffolk and North Essex NHS Foundation Trust's REACT service is looking for Healthcare & Therapy Assistants to support patients at home, helping them regain independence and avoid hospital admissions. The role involves visiting patients across East Suffolk and working various shifts to cover weekdays, weekends and bank holidays.

You will operate as part of a multi-disciplinary team, with training provided across nursing, therapy, and social care.

Responsibilities

• Provide one-to-one support with patients in their home environment, assisting with personal care and daily activities using a reablement approach.
• Travel across East Suffolk to reach patients on the caseload; a full UK driving licence and access to a vehicle is essential; mileage reimbursed.
• Develop and maintain a flexible, proactive and positive approach to the role, while being innovative and open to change in working practices.
• Take responsibility and ownership of high-quality service delivery, and support ESNEFT's values.
• Manage an effective interface with colleagues from health, social care and voluntary care services to ensure integrated patient-centred care.
• Support clinicians by completing delegated tasks and gaining skills through training across Nursing, Occupational Therapy, Physiotherapy, Podiatry, Dietetics, Mental Health and Social Care.
