Job summary

Central London Community Healthcare NHS Trust in London seeks a Band 8a Primary Care Network Pharmacist to support Hammersmith & Fulham North PCN. You will join our pharmacy team to provide specialist medicines expertise and promote safe, effective, person-centred medicine use.

The role covers conducting structured medication reviews, developing prescribing clinics, and advising GPs, nurses and care teams. You will travel between GP practices, contribute to medicines management, and drive

Qualifications

Relevant post-registration experience in GP practice/primary care.
Postgraduate qualification(s) including independent prescribing, DPP and diabetes-related expertise.
Strong knowledge of prescribing and medicines management in primary care.
Sound clinical decision-making and problem-solving skills across a broad range of conditions.
Effective communication and relationship-building with patients and health teams.

Responsibilities

Conduct structured medication reviews, including for people affected by inappropriate polypharmacy or overprescribing.
Develop and deliver prescribing clinics supporting the management of long-term conditions.
Provide specialist medicines advice to GPs, nurses, community pharmacists, care coordinators, carers, families and other health and social care professionals.
Contribute to Trust-wide medicines management, including policies, patient safety initiatives, medicines alerts and professional training.
Work across GP practices within Hammersmith & Fulham North PCN, including lone working and travel between sites.
