Qualifikationen

• Registered pharmacist with UK registration.
• Experience in primary care/pharmacy practice preferred.
• Willingness to develop towards Independent Prescriber.

Aufgaben

• Provide structured medication reviews for patients with complex needs.
• Support medicines optimisation across the PCN and safety initiatives.
• Provide medication advice to patients, carers and clinicians.
• Support chronic disease management through clinical reviews and monitoring.
• Contribute to safety alerts, audits and quality improvement work.
• Work with GPs, nurses, technicians and care coordinators as a team.
