Hounslow Family Practice is seeking a qualified pharmacist with independent prescribing or CPPE completion to join our multidisciplinary team, delivering patient-centred medicines optimisation within primary care.

You will conduct medication reviews, manage repeat prescriptions, and lead on pharmaceutical governance while collaborating with GPs, ANPs, and practice staff to improve safety and outcomes.

Qualificações

• Must hold MPharm or equivalent and be registered with General Pharmaceutical Council (GPhC).
• Independent prescriber or CPPE completion is essential.
• Experience in primary care and strong therapeutic knowledge are required.
• Excellent written and verbal communication skills with patients and colleagues.

Responsabilidades

• Conduct face-to-face clinical medication reviews for polypharmacy, elderly, and complex patients.
• Deliver a repeat prescription service and authorise repeats and acute prescriptions.
• Monitor medication adherence and promote shared decision-making and self-care.
• Set up long-term condition clinics and manage acute/minor ailments.
• Respond to medicine-related enquiries from staff, patients, and community pharmacies.
• Collaborate with ICB pharmacists, PCN pharmacy teams, and wider health networks.
