Job summary

Leaders in Care is recruiting an experienced Senior Palliative Care Nurse to join a specialist palliative care service covering Berkshire West and South Oxfordshire. You will work autonomously within a supportive multidisciplinary team, delivering high-quality end-of-life and symptom management care.

The role offers full-time or part-time hours, enhanced pay for unsociable hours, 37.5 hours full-time or 22.5 hours part-time, 35 days annual leave, and funded Non-Medical Prescribing training with

Qualifications

• Active UK NMC registration (RGN/Adult) required.
• Experience in palliative or end-of-life care.
• Strong clinical assessment and decision-making skills.
• Excellent communication skills.
• Full UK driving licence and own vehicle.
• Ability to work autonomously within a multidisciplinary team.

Responsibilities

• Completing holistic assessments and developing individualised care plans.
• Managing complex symptoms and supporting patients approaching end of life.
• Providing specialist advice to patients, families, carers and healthcare professionals.
• Supporting telephone triage and responding to clinical enquiries.
• Working closely with GPs, community teams and the wider multidisciplinary team.
• Advocating for patients and ensuring compassionate, person-centred care.
• Maintaining accurate and high-quality clinical documentation.
