Leaders In Care Recruitment Ltd in Berkshire West / South Oxfordshire is seeking an experienced Palliative Care Nurse, Hospice Nurse or Community Nurse for a Senior Palliative Care Nurse role. You will provide specialist assessment, symptom management and clinical support to patients with life-limiting illnesses, working autonomously within a supportive multidisciplinary team.

Qualifications

• Active UK NMC registration as an RGN/Adult Nurse.
• Previous palliative or end-of-life care experience.
• Strong clinical assessment and decision‑making skills.

Responsibilities

• Completing holistic assessments and developing individualised care plans.
• Managing complex symptoms and supporting patients approaching end of life.
• Providing specialist advice to patients, families, carers and healthcare professionals.
• Supporting telephone triage and responding to clinical enquiries.
• Working closely with GPs, community teams and the wider multidisciplinary team.
• Advocating for patients and ensuring compassionate, person-centred care.
• Maintaining accurate and high-quality clinical documentation.
