Job summary

St Luke's Hospice Basildon & Thurrock is seeking an experienced Palliative Care Clinical Nurse Specialist to lead and deliver expert palliative care across the community and local hospice services.

You will provide autonomous, advanced clinical practice, manage a caseload with complex needs, collaborate with GPs, specialists and voluntary services, and support patients and families at home or in hospice settings. 37.5 hours/week over four days, with a 12-month maternity cover.

Qualifications

• First-level registered nurse with post-registration experience in palliative care.
• Experience delivering community-based care with autonomous practice.
• Ability to co-ordinate care with MDTs and external partners.

Responsibilities

• Lead specialist and provide expert palliative care in the community and 7-day services.
• Manage a caseload with patients with complex needs and their families.
• Liaise with referrers, primary/secondary care, and voluntary sectors to deliver integrated care.
