Job summary

St Luke's Hospice in Basildon invites an experienced Palliative Care Clinical Nurse Specialist to join our Hospice Community Service on a 6-month temporary contract. The role offers 37.5 hours over four days, with occasional weekends and a focus on delivering expert palliative care in the community.

You will lead specialist care, manage complex caseloads, liaise with multi-disciplinary teams, and support families.

Qualifications

• Must be a first level registered nurse.
• Significant post-registration palliative care experience at Band 6/7 or above.
• Ability to work independently in the community setting.
• Car driver with a current valid driving licence.

Responsibilities

• Lead specialist and source of expertise in palliative care within community services.
• Manage a caseload of patients and their families with complex needs at home.
• Collaborate with Hospice staff, primary/secondary care professionals and voluntary sectors.
• Provide holistic assessments and plan levels of involvement for each situation.
• Coordinate care across services and participate in MDT/GSF meetings.
• Deliver high-quality information to patients and carers about disease, treatment and care.
