Job summary

North Cumbria Integrated Care NHS Foundation Trust is seeking a Community Clinical Nurse Specialist in Specialist Palliative Care to provide expert assessment, advice and care coordination for patients with advanced illnesses in the community. The CNS will work within a multidisciplinary team and with GP practices to support personalised end-of-life care and education across the service.

The role is based with the Community CNS Specialist Palliative Care Team in the west of the county, with

Qualifications

NMC registration as an adult nurse.
Postgraduate qualification in Specialist Palliative Care or willingness to undertake.
Degree in Nursing or related subject.
Evidence of continuing professional development (CPD).
Practice Assessor/Supervisor qualification or willingness to achieve.

Responsibilities

Provide specialist nursing assessment, advice and support for patients with complex palliative and end-of-life needs.
Undertake holistic assessments and develop individualised care plans.
Provide specialist advice on symptom management (pain, breathlessness, nausea, anxiety, agitation).
Support advance care planning and care discussions.
Provide education, supervision and clinical support to healthcare professionals.
