Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting an experienced paediatric nurse to join the Community Outreach Assessment and Support Team (COAST) working across Portsmouth and South East Hampshire. You will assess acutely unwell children by telephone, virtual and face-to-face methods, delivering safe and effective community care with autonomous decision-making.

Qualifications

• Deliver specialist nursing care to babies, children and young people with acute and short-term healthcare needs.
• Coordinate care and support timely discharge through urgent community pathways.
• Assess acutely unwell children and provide safe, evidence-based interventions in the home or community.

Responsibilities

• Deliver specialist nursing care, support and care coordination for babies, children and young people with acute and short-term healthcare needs.
• Support admission avoidance and facilitate safe, timely discharge through urgent care pathways in the community.
• Undertake telephone, virtual and face-to-face assessments, including autonomous assessment in home environments.
• Manage a caseload of children and young people, planning and delivering evidence-based care and interventions.
• Recognise deterioration, initiate appropriate actions and escalate to acute paediatric services as needed.
• Collaborate with families and MDTs to enable care at home where appropriate.
• Provide clinical supervision and development opportunities for junior staff with the Clinical Team Leader.
• Maintain high standards of practice through ongoing professional development.
• Participate in clinical supervision, safeguarding supervision, reflective practice and quality improvement.
