Job summary

Southern Health NHS Foundation Trust is seeking an experienced paediatric nurse to join the COAST across Portsmouth and South East Hampshire, delivering nurse-led community assessments for babies, children and young people with acute and short-term needs. You will work closely with hospital paediatric teams, GPs and MDT colleagues to avoid unnecessary admissions and support early discharge.

Job description

We have an exciting opportunity for an experienced paediatric nurse to join our Children's Outreach Assessment and Support Team (COAST) across Portsmouth and South East Hampshire.

COAST provides responsive, nurse-led care for babies, children and young people with acute and short-term healthcare needs. Working closely with our Acute CCN colleagues, hospital paediatric teams, GPs and wider MDT, we assess and support children in the community, helping to avoid unnecessary hospital admissions and facilitating early discharge wherever possible.

You will undertake telephone, virtual and face-to-face assessments of acutely unwell children, using your clinical expertise and decision-making skills to deliver safe, effective care. You will work autonomously whilst being supported by an experienced and highly skilled team with access to specialist paediatric advice.
