Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting an experienced paediatric nurse to join the Community Outreach Assessment and Support Team (COAST) working across Portsmouth and South East Hampshire. You will assess acutely unwell children by telephone, virtual and face-to-face methods, delivering safe and effective community care with autonomous decision-making.
Join a supportive, nurse‑led team with access to specialist paediatric advice.
We have an exciting opportunity for an experienced paediatric nurse to join our Children's Outreach Assessment and Support Team (COAST) across Portsmouth and South East Hampshire.
COAST provides responsive, nurse-led care for babies, children and young people with acute and short-term healthcare needs. Working closely with our Acute CCN colleagues, hospital paediatric teams, GPs and wider MDT, we assess and support children in the community, helping to avoid unnecessary hospital admissions and facilitating early discharge wherever possible.
You will undertake telephone, virtual and face-to-face assessments of acutely unwell children, using your clinical expertise and decision-making skills to deliver safe, effective care. You will work autonomously whilst being supported by an experienced and highly skilled team with access to specialist paediatric advice.
This is an excellent opportunity to develop acute assessment skills, expand your community experience and be part of an innovative service delivering care closer to home.
If you are passionate about improving outcomes and want to be part of a dynamic and evolving service, we would love to hear from you.
