Job summary

Cumbria, Northumberland, Tyne and wear (CNTW) NHS Foundation Trust is recruiting for a Band 6 Community Practitioner to lead a new project within the Northumberland Recovery Partnership. The post will work alongside the Northumberland Alcohol Outreach team and run for two years with potential extension.

You will undertake complex assessments, coordinate care with local housing projects, and deliver trauma-informed, person-centred interventions to support recovery and reduce risk.

Qualifications

• Registered nurse with current practice and registration.
• Post-registration training in clinical/psychological interventions.
• Experience in community settings; safeguarding/MAPPA knowledge.

Responsibilities

• Engage individuals housed in local housing projects to support recovery.
• Coordinate services and multi-agency care; manage risks.
• Provide compassionate, person-centred care using psycho-social interventions.
