Job summary

CNTW NHS Foundation Trust is seeking a Band 6 Community Practitioner to lead a new project within the Northumberland Recovery Partnership. The role involves working with housing projects, conducting assessments, and coordinating services to support recovery.

You will collaborate with clinical leads, provide interventions, and support staff in delivering compassionate, person-centred care within a multi-disciplinary team. Fixed-term for 2 years with potential review.

Qualifications

• Registered nursing qualifications with current registration (RNMH/RMN/RNLD or equivalent AHP/Clinical Psychologist).
• Post-registration training in clinical/psychological interventions and Older Adults pathways.
• Experience in trauma-informed care, safeguarding and MAPPA.
• Up-to-date knowledge of relevant legislation and non-medical prescribing.

Responsibilities

• Lead and deliver Assertive Outreach within Northumberland Recovery Partnership.
• Engage individuals in local housing projects and coordinate support services.
• Conduct comprehensive assessments, care planning, formulation and multi-agency collaboration.
• Provide harm-reduction guidance and psycho-social interventions to support recovery.
