Responsibilities

• Conduct outcome-focused assessments for adults with physical disabilities and age-related conditions.
• Perform complex moving and handling assessments within the Front Door Team.
• Evaluate physical dysfunction to recommend equipment solutions.
• Provide expert manual handling advice and intervention planning.
• Support safe and effective care planning and manage cases independently.
• Collaborate with multidisciplinary teams and partner agencies for holistic care.
• Maintain meticulous records and assessment documentation.
