Job summary

HCRG Care Group is recruiting a Specialist Occupational Therapist to join a newly mobilised Intermediate Care and Reablement Service in Medway. You will deliver specialist assessment, rehabilitation and reablement to help people regain independence and stay at home, working with a multidisciplinary team.

You will lead reablement pathways, contribute to service development and ensure high-quality care while supporting discharge planning and personalised plans for patients.

Qualifications

• Degree or Diploma in Occupational Therapy.
• Current HCPC registration.
• Significant post-registration Occupational Therapy experience.
• Experience working at Band 6 level or above.
• Experience in rehabilitation, intermediate care, community services or similar.
• Experience managing complex clinical caseloads.
• Excellent communication, organisation and prioritisation skills.
• Full UK driving licence and access to a vehicle for work.

Responsibilities

• Provide specialist assessment, rehabilitation and reablement for a wide range of conditions.
• Work autonomously across community and intermediate care settings.
• Develop personalised rehabilitation plans and coordinate care with patients, carers and colleagues.
• Provide leadership and support across the therapy team and service development.
