Job summary

HCRG Care Group is launching a brand-new Intermediate Care and Reablement Service in Medway. We are seeking a Specialist Occupational Therapist to lead rehabilitation pathways, support discharge planning, and help shape service development within a multidisciplinary team.

You will assess, plan and deliver person-centred rehabilitation to enable people to recover, regain independence and remain at home. A Band 7 salary and NHS pension are offered with excellent professional development

Qualifications

• Degree or Diploma in Occupational Therapy required.
• Current HCPC registration essential.
• Experience at Band 6 level or above in rehabilitation settings.
• Experience in multidisciplinary teams and complex caseloads.
• Excellent communication, organisation and prioritisation skills.

Responsibilities

• Provide specialist assessment, rehabilitation and reablement for complex conditions.
• Work autonomously across community and intermediate care settings.
• Develop personalised rehabilitation plans and coordinate care with patients and carers.
• Support discharge planning and home adaptations as needed.
• Provide leadership and development support across the therapy team.
