Job summary

Oxfordshire County Council’s Home First Team is seeking an experienced Occupational Therapist to support people after hospital discharge, enabling independence at home. You’ll perform assessments, remove barriers, and implement practical solutions using OT skills from reablement to assistive technology, while balancing risk and safety.

Qualifications

• Qualified Occupational Therapist with HCPC registration.
• Strong communication skills and ability to work in multidisciplinary teams.
• Committed to flexible, person-centred care.

Responsibilities

• Conduct timely assessments and identify barriers to independence.
• Develop practical, person-centred solutions and promote wellbeing, choice and control.
• Utilise OT skills including functional assessment, reablement, equipment and assistive technology, and moving and handling.
• Manage and prioritise own caseload and undertake community visits, balancing independence with risk.
