Job summary

Midlands Partnership NHS Foundation Trust in Staffordshire is recruiting a Senior Occupational Therapist to join the Home First Service (Discharge to Access Pathway) based in Leek. You will work within an integrated health and social care team to assess and support patients in their homes to promote independence and safe hospital discharge.

You will lead on functional assessments, develop personalised treatment plans, and collaborate with multidisciplinary teams to prevent admissions.

Qualifications

• Degree in Occupational Therapy and HCPC registration.
• Fieldwork educators accreditation.
• Masters level modules.
• Evidence of continuing clinical study.
• APPLE accreditation.
• BAOT / COT membership.

Responsibilities

• Assess new service user referrals and care needs.
• Develop and implement treatment/care plans.
• Promote independence and safe discharge.
• Collaborate with MDT to assess and manage conditions.
• Monitor outcomes and adjust plans.
