Job summary

Nottinghamshire Healthcare NHS Foundation Trust in the Rushcliffe Community Therapy Service is seeking an experienced Occupational Therapist to join our forward looking team delivering care in people’s homes and local community settings.

You will work autonomously and as part of a multidisciplinary team with physiotherapists, support staff, district nurses and social workers to promote recovery, independence and safe self-care.

Qualifications

• Qualified Occupational Therapist registered with HCPC.
• Experience in community settings preferred.
• Full UK driving license required.

Responsibilities

• Visit patients in their homes to maximise independence and safety.
• Work within a multi-disciplinary team to coordinate care.
• Provide community-based occupational therapy assessment and rehabilitation.
• Support timely discharges and falls management.
