Nottinghamshire Healthcare NHS Foundation Trust in the Rushcliffe Community Therapy Service is seeking an experienced Occupational Therapist to join our forward looking team delivering care in people’s homes and local community settings.

You will work autonomously and as part of a multidisciplinary team with physiotherapists, support staff, district nurses and social workers to promote recovery, independence and safe self-care.

Qualifications

  • Qualified Occupational Therapist registered with HCPC.
  • Experience in community settings preferred.
  • Full UK driving license required.

Responsibilities

In your new role you will work closely with Physiotherapists, Assistant practitioners, support staff, district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.

We are working towards a 7 day service - 8am to 8pm on rota basis. Self rostering is encouraged.

An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, palliative care support and support timely discharges from our acute Trust partners.

You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient’s independence, safety and quality of life.

The post will involve cross site working and community visits.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change.
