Marie Curie is seeking experienced Senior Nurses to develop a new and exciting palliative integrated discharge service from acute sites across Cwm Taf Morgannwg University Health Board. Apply today for a rewarding career where you can help shape the future of palliative care within the region!
The new service will involve Senior Nurses proactively identifying patients approaching the end of their lives, ensuring supportive conversations are had to discuss their preferences and wishes. Working with the Health Board's existing teams and patients' loved ones, Senior Nurses will undertake holistic assessments to support fast-track assessments and ongoing care and treatment plans, including advance care planning and the completion of clear treatment escalation plans. They will work as part of the discharge team, collaborating with multidisciplinary and palliative care teams to ensure safe and timely discharges to appropriate care settings, respecting patient preferences and needs. Additionally, Senior Nurses will coordinate with Marie Curie's Hospice Care at Home Service to facilitate community discharges.

Salary: Marie Curie Clinical Pay Scale Band 6 (£37,898 - £45,637 pro-rata per annum + enhancements).
Contract: Permanent, full-time and part-time options available. Service operates 09:00-17:00, Monday to Friday.
Location: Based at Ysbyty Cwm Rhondda (Discharge Hub), with flexibility to work across the Health Board area as required.
