Job summary
Berkshire Healthcare NHS Foundation Trust is seeking an Advanced Specialist Mental Health Pharmacist to lead medicines optimisation across BHFT mental health and community services. Qualifications
- Masters in pharmacy or equivalent.
- Statutory registration with GPhC.
- Clinical or psychiatric postgraduate certificate.
- Clinical or psychiatric postgraduate diploma.
- Independent prescriber.
Responsibilities
- Develop, plan, manage and lead a high quality medicines optimisation service for CPE and BHFT teams.
- Provide expert medication referrals and advise healthcare professionals on prescribing and changes.
- Collaborate with GP practices to ensure appropriate supply and monitoring of medicines.
- Lead clinical audits to improve service delivery and patient care.
- Support processes and policies for medicines prescribing across BHFT.
